Voucher form

a) Voucher form along with vouchers meant for payments deliver personally to the Sodexo subsidiary where you have signed your contract or send it by post in the form of avaluable letter or in other safety way to the address

Radlická 2, 150 00 Praha 5 (never send the vouchers only by registered mail).  
 
b) Please use an original form that we will send you along with an invoice and your printed initials.
